Education, Health & Care Plan (EHCP)

Education Health Care Plan (EHCP) is the statutory document that replaced pupils’ Statement of SEN under the 2014 legislation. Every year an Annual Review of the EHCP is held for each student. This is an opportunity to review progress and consider the document, including Outcomes and Provision Plans. As part of this process, suggested changes to the EHCP may be sent to the maintaining local authority (LA), if appropriate. Local authorities are responsible for maintaining EHCPs and reissuing them where appropriate.

Key educational components of the ehcp

  • Section B
Describes the student’s Special Educational Need  (SEN)
  • Section E
Outcomes, which are the overarching targets for the current key stage for each area of Special Educational Need in Section B
  • Section F
Provision Plans: which set out resources and strategies that will be available to help achieve the outcome. These also set small step targets towards the outcome for the current year, which are evaluated at the end of Terms 1, 3, and 5. New Provision Plans are put in place for each student every year, in agreement with parents/carers.

Meetings are offered to parents/carers three times per year to discuss the Provision Plans and progress against them (two parent consultation meetings and the Annual Review).

To be as flexible as possible, Annual Reviews are offered in person, through a virtual meeting (Googlemeet) or via the telephone. Any professionals you tell us about who are involved with your child will be invited to take part or submit a report to inform the discussion around your child’s EHCP.

At the beginning of the school year, a form is sent home for parents to inform the school of any professionals, e.g. Social Worker, Respite, therapists etc, that you wish to invite to the Annual Review. You can also contact us at any time to advise of any changes.

If you have any questions about your child’s Annual Review etc., please contact Chantell Mayers on 01474 709420, extension 335.
